Doc, My Thumb is painfull
Patient's characteristic are different. There are talkative patients. There are silent ones. Sometimes the talktative ones made me peevish. Like my 40 years old man patient. As a normal human I dont like his attitude. He always talk with hard voice. And he always doesnt believe what doctor said.
Just call him Zopi. He came again about 2 weeks ago with symptom pain in his right thumb nail. I saw his nail was almost detached from the place. I told him that it had to be released. He didnt reject. But he didnt accept it too.
It's what I regret. I didnt ask his agreement to take the nail. I admitted I forgot. Then he came again 3 days ago for other symptoms that were cough. Actually I asked him to control. He didnt do that. I saw the thumb had been recovered. The nail had regrew again. But it stil havent covered all part of the nail place.

Sex topic always become a funny topic
I don’t know why everything about sex behaviour and sex thing always make me feel funny. On television, sex topic is one of comedian’s “weapon” to make the audience laugh. I am sure most of you agree about this. I don’t know whether this a high quality comedian topic or village topic (Tukul’s term : katro topic)
Talking about sex behaviour, sometimes I got sexual transmitted disease in my clinic. Just now, I had a woman patient who always check her urine periodically to know whether she got a disease or not. For us, as medical worker, directly we can think that the patient wasn’t “a good woman”. Yeah, In Indonesia, free sex was still not accepted for most people. So we will not enjoy associating with the people doing free sex.
It’s sometimes difficult to ask the sexual status of someone. I mean it’s hard to ask them, whether they had been had a dirty sex, they always changed their partner sex, or whether they had a job as a whore or not. If we have succeeded to get the answer, it was always hardly to believe. That’s because it was a private thing. And sometimes they lied to cover the truth.
After I accepted the laboratory value of her urine, I got an infection sign. There was blood in the urine and the red cell blood increased. I asked her the question if she always did dirty sex or not. She denied. She said, her husband was the one who always played to the whorehouse. And she couldn’t prevent her husband to do that. What a “great” household!
Whatever! I don’t really care with who was wrong! Because It’s their bussiness. It was outside my control. Then I advised her to correct her sexual behaviour. And I teached her a little knowledge about healthy sex, although I was really sure that he must know about that. That was an advice to wear condom as a protection tool to sexual transmitted disease. Actually, I frightened her a little about the danger of sexual disease. I guees it was one of my partitipation to prevent more sexual disease transmission. Ha ha. I thought it worked little. Because she told me that she would show the laboratory value to her husband.
Not just that. Before, I have accepted other sexual disease patient. Men and women. The men always came and registered with “headache”. After they met me they told the truth that they got a “younger disease”. If they said so, I directly know that they got a gonorrhea with the symptom suppuration in his urine. The women sometimes got the leucorrhea that was a white discharge came out from the v****a. I always do a same thing with them. Teaching a healthy sex. I asked them to avoid having sex with a whore or If they cant stand to do that, use condom protection.
The stories behind them were different. The reason for woman because of the need to get fast money. And for man, because they couldn’t hold out the desire. Like I said before, the sex story of them sometimes can make laugh. One of the story was from the whore woman. She said her v****a was very painfull. When I asked why, she frankly said she had a sex with her customer last night. What made me almost have a very big laugh when she said. “I have never seen a large p***s like that. Very very hugh.”

Not sleep for patient
I got a critical patient two night ago. He got a traffic accident just before my clinic at midnight. So many people from around the clinic came in a group enter the clinic just to watch the victim. I always feel angry if it happened. It’s not a movie! It was a bad thing to watch. But that was it that always happened in most hospital in Indonesia. The people always have a big curiosity to know so sometimes nobody can prevent them to find out.
Back to the victim. He was not conscious. There was torn wound at his lip and his right chin. I checked his breath and his pulse. It was fine. Directly I set a neck collar to prevent his neck move. I was afraid there was fracture at his neck.

I had been trying to make hime aware by calling him, by giving a pinch. He kept silent. I tried to find his identity card. Oh, god! No wallet. No card! How to contact his family! He needed his family to help him and to help me as a helper.
An old man who also took a look at this patient said “I know this man!” Then He got out. A few minutes latter, a group of women came in hurry to look at the patient. And then they are crying! Thanks god! The family had come.
I knew his name from his family. Just call him Mus. Mus was still unconsciuos.From the smell, I was so sure he had been drinking alcohol and then drove a motorcycle. Wherever in the world, driving on drunk was forbidden. But it had been happened. The law would affect him after he got well. Now, I would be the one who affected his life or his death!
I kept trying to wake him up. Still no change. The blood tension had decreased to 80 mmHg. I instructed my nurse to set an infusion. The family was crying to see the man. I tried to shut them up. I told them to be patient and pray.
“He has a problem with his wife, doctor. It has been 3 days he doesn’t get home.” Said his sister.
I turned to fix all the thing I could do. I sewed the lip. One little part of the lip almost separated from the center one. It was quite difficult to sew. After that, I moved to the chin wound. It’s torn to, about 5 centimetres. While I sewed, the nurse measured his blood tension again. I had been raising up to 130/80 mmHg.
His sister tried to make him talk. She did it! The man could answer her talk. It’s a good sign. Thanks god!
Ok, The man had been stable. He needed a longer observation. My place wasn’t the right place for him again because it was not a hospital. I sent him to the public hopsital in my city that night.
Oh, man. I realized it had been 4.30 a.m. No sleep this night. And I still had to take an assignment to receive patients this morning until 2.00 p.m. It’s good!

Treat burned wound
My patient with a burned wound, just call him Budi, came again this morning. Budi was a man about 40 years old. He came to control his wound. More than one and a half weeks ago he got an explosive stove in his food stall. His both lower arm and his face got burnt but just at the outer layer. I remember he screamed when he came first time because of the pain. I could imagine how painful it is!
First time, I used Burnazin® zalf, zalf with silver sulfadiazin. I was quite sure that it’s the best local medicine for the burned wound. I gave him anti inflammation and antibiotics too. A few days later, he came again to control. I saw, the wound didn’t get better. It was covered with dry zalf and suppuration under it.
I changed my mind. Obviously Burnazin wasn’t the best medicine for this wound. It was sticky to the wound and made painful to withdraw it.Then I moved to another antibiotic zalf. It was called Bioplacenton®, contained neomycin and bioplasenton. After it was cleaned and lubricated by the zalf, I wrapped it with bandage.
Then he came again this morning. It was better. Bioplacenton didn’t stick to the wound and it made easy to withdraw the bandage then with Burnazin. I thought it was a good experience for me. Being a doctor was sometimes related with experiences, not just from learning from the book.
This morning, I saw the wound. It was better. The wound slowly covered with the normal layer from the wound side. I was sure it would need just a few days again to recover. I really hoped it would be fast. Because I felt pity to see him come and come again to control.
Health is so expensive. Isn’t it?

Surging lip cyst
It was a little shocking day for me this day. When I treated a cysted lip patient, just before I wanted to inject the anaesthetic drug to the lip, a little girl of 10 years old, came with fever seizure. The kid was stiffened. Her eyes and head was retracted upside. There was amount of rice inside her mouth. I kept trying no to panic.
With 3 nurses, I turned my focus to the kid. I instructed many thing to my nurses. From giving the anti seizure from her anal, sucking the vomit, giving the oxygen, and giving infusion. We did it! My team and I got succed to treat the kid. The kid finally stopped stiffening. She felt sleep because of the drug entered the anal.
I was so grateful for this. The longer the seizure, the more dangerous the effect. People with seizure can hardly breath. If it continued more than 5 minutes, the brain could be damaged. And then the kid could be an idiot girl. What a horrable it is!
While observating the kid, I turned back my focus to Agus, the man with cysted lip. He had come three days ago for this disease, but I was not directly surge him. Because of no change in his disease after taking my medicine, he came again.
It was my first experience to surge cyst in the inside lip. It had about 1 cm of diameter. After the anaesthetic drug worked, slowly but sure I cut the tissue. The cyst directly showed up. It contain white jelly. With the assistance from 2 nurses I separated the cyst with the body tissue.It was not an easy thing. I intended to take it out as the ball form. Unfortunately, it broke down. The jelly liquid cames out slowly. I decided to cut it piece by piece. Finally the whole cyst disappeared. I closed the operation by sewing it with absorbable suture.
What a great experience for me it is!

Curcuma was proven to my patient
Doctors’ feeling are often affected by their patients’ condition. A doctor will feel happy if the patient can be recovered from his medicine. It’s the greatest happiness to become a doctor. On the contrary, a doctor will feel sad if the patient doesn’t heal and become worse after taking the medicine. It’s the hughest sadness of a doctor. I always feel the experience too since becoming a doctor two years ago.
This morning, July 11th 2008, I met a patient, just call him Syamsuddin, with little liver dysfunction. The laboratorium value of his liver were 116 U/L for SGOT and 136 U/L for SGPT on June 27th 2008. It should be under 40 U/L for the best result and under 60 U/L to continue his job in a mining company.
He had been under treatment for 2 month for this liver. From general physician until internist. But the liver didn’t get better. He was almost frustrated. He came to me as the last doctor. If a patient had met a speciality doctor, I always had an uncomfortable feeling to him. A speciality doctor was incapable to cure him, how could I do? But I shouldn’t give up.
Knowing his result still bad, he begged me to help him to manipulate the result. He said he had been almost 2 month cant get his previous job because of this. He had a family to feed. Of course I rejected his request. I told him that the rule was made for the workers’ goodness. The high level meant the liver was not good to take for work. The work, especially mining work, need a good liver. If the liver was forced to work, the liver will get worse. Then it will need more money to treat the damage.
He understood what I encouraged. He would follow my advice. I felt pity toward him. But I thought I had done the right thing. I advise him to take a total rest. Not to do weight activities. Avoid eating and drinking chemical contained food. And I gave him a hepatoprotector medicine, Curcuma to repair his liver function.
After 2 weeks, he came again to check his laboratorium value. Actually I advised him to take a medication for 1 month. But he had came in 2 weeks. Ok, It’s his right to make a laboratorium check.
Alhamdulillah, the result was better. The value had decreased to 58 U/L for SGOT and 56 U/L for SGPT. More than 50 % better. I was so happy to know that. It’s just like myself getting better health. Even though it’s not really normal, I was glad to know that the man can work again in his company.
What he said next make me happier. “I am so happy, doctor. I want to cry to heard this news. Thanks doctor.”

Have you ever got what you ever dream? I myself have proved that sometimes what we dream can be real. One of them is working in Borneo, Indonesia. It has been a long time I want to go and work there. And now, I got it. I am in Sangatta, East Kutai, East Borneo, Indonesia.
This fact make me believe that everybody can get what he dream. Dont afraid to dream. I have ever read a proverb from the west. "Be careful with your dream, because it can happen."
Now I believe that proverb. But I think we should write what we want. I did it. I dreamt to have notebook, communicator, etc. There is a list full with my dreams I have written.
One by one my dream come true. I have a dream to live and work in Europe. I am not patient to get it!

Warga Jakarta yang mengandalkan angkutan umum sebagai alat transportasi pastilah sering berinteraksi dengan pengamen. Kurangnya lapangan pekerjaan di sektor formal dan padatnya ibukota membuat banyak orang kreatif menciptakan apa saja yang bisa dijual untuk bisa hidup,
termasuk suara. Ratusan pengamen menjual suara di sarana angkutan umum di Jakarta setiap hari, mulai dari kereta api, bus, dan angkot. Saya sebagai pengguna alat transportasi publik di Jakarta adalah “penonton tetap” para pengamen ini.
Tiada perjalanan tanpa kehadiran pengamen. Itulah mungkin istilah yang tepat untuk menggambarkan kehadiran pengamen di Jakarta. Hampir semua kendaraan umum setiap hari disinggahi penjaja suara ini untuk mendapatkan sekedar uang receh dari penumpang. Dalam satu lintasan, kadang perjalanan bisa dihibur sampai lima pengamen secara berganti-gantian.
Hal ini saya alami sendiri dalam perjalanan saya dari Cibinong ke UKI melalui tol Jagorawi, 13 Desember 2007 silam. Memang tidak semua pengamen memiliki modal suara yang bagus. Ada juga yang pas-pasan atau bahkan fals tapi nekad menjual suara. Tapi saya beruntung kali itu karena bisa menikmati beberapa suara emas para penyanyi jalanan.
Dimulai saat bus ber-AC Laju Utama yang saya naiki sedang ngetem (berhenti untuk menunggu datangnya penumpang).
Di tengah nyamannya bis yang sesaat lagi akan melaju ke UKI, tiga orang pemuda naik dengan membawa perlengkapan bandnya. Satu orang menabuh drum, satu memegang bas, dan satu lagi memetik gitar. Sang gitaris yang juga vokalis membawakan dua lagu dengan sangat indah yaitu “Sebelum Cahaya” karya Letto, dan “11 Januari” karya GIGI. Tampak banyak penumpang cukup menikmati irama lagu mereka. Karena penampilan mereka cukup baik dan itu adalah penampilan pengamen pertama di bus itu maka cukup banyak yang mengapresiasi dengan memberikan uang receh kepada mereka.
Setelah mereka turun karena bus sudah bersiap masuk ke pintu tol, seorang pengamen solo muncul lagi. Rupanya pria ini sudah menunggu giliran untuk menyanyi dari tadi di kursi belakang. Dengan berbekal sebuah gitar, pemuda bertato naga di lengan kanannya ini menghibur para pengunjung dengan suaranya yang cukup merdu. Dia menyanyikan tiga lagu yang cukup populer. Salah satunya adalah “Oemar Bakri” milik Iwan Fals. Perjalanan ke UKI yang menempuh waktu sekitar setengah jam ini menjadi tak terasa dengan kehadirannya.
“Saya cukup menikmati lagu-lagu tadi. Tapi sebelumnya kadang saya terganggu juga kalau lagi bete.” Kata Dewi, seorang gadis manis yang juga turut dalam perjalanan tersebut.
Akhirnya perjalanan saya ke UKI sampai juga. Saya pindah ke bis non AC ke arah terminal Bekasi. Lagi-lagi saya mendapati seorang pengamen beraksi di bus itu. Seorang pengamen berkumis tipis yang bernama Arifin dengan berbekal sebuah gitar listrik menghibur penumpang. Penampilan Arifin cukup unik. Dia membawa gitar yang dilengkapi sebuah mikropon kecil yang dilekatkan pada gitarnya dan perangkat speaker kecil yang dimasukkan ke dalam tasnya. Dengan modal itu, Arifin mengawali “konsernya” dengan lagu “Beautiful Girl”. Lagu lama memang tapi di tangan Arifin lagu itu menjadi sangat nikmat.
Suaranya yang indah bisa terdengar dengan jelas ke seluruh penjuru bus berkat kehadiran speaker kecilnya. Lagu keduanya berjudul “Munajat Cinta” milik The Rock juga tidak mengecewakan. Akhirnya konsernya ini ditutup oleh lagu “Dust in the Wind”. Kesan perjalanan kali ini yang agak gerah dan pemandangan dalam bus yang kurang bersih menjadi lebih terhibur karena lagu-lagu dari Arifin.
“Sudah empat tahun saya mengamen. Sekarang saya sudah jenuh.” Kata Arifin saat saya mendapatkan kesempatan mewawancarainya. Pria beranak satu ini mengatakan memilih jadi pengamen karena tidak ada pilihan lain. “Dari pada mengemis mending ngamen.”
Dia lalu menceritakan kehidupannya selama menjadi pengamen. Setiap hari Arifin selalu menaiki sampai 6 bus untuk menjual suara. Dia lebih sering memilih bus Jurusan UKI-BEKASI karena tempat tinggalnya berada di Rawa Panjang, Bekasi. Jam kerjanya kebanyakan sore hari. Dia mulai naik bis jam 5 sore, dan pulang jam 9 malam. Walaupun termasuk pekerjaan golongan bawah, ternyata penghasilannya melebih nilai upah minimum regional Jakarta.
“Saya bisa mendapatkan 40 sampai 50 ribu dalam sehari. Tapi sekarang agak seret karena munculnya Perda baru yang melarang adanya pengamen.” Katanya mengeluh. “Tapi itukan cuma peraturan. Ya, buktinya masih banyak teman-teman saya yang bebas ngamen.”
Lain Arifin lain lagi pengamen perempuan yang saya temui dua hari setelahnya saat saya naik metromini jurusan Bogor-Kampung Rambutan. Di situ saya mendapati pengamen yang bersuara fals. Dalam pengantar ngamennya, dia memohon rezeki dari para penumpang agar bisa menyekolahkan anaknya. Dia kemudian menyanyikan potongan-potongan lagu yang dicampur-campur mulai dari lagu milik Ungu, Rhoma Irama dan banyak lagi.
Nada suaranya dan iringan musik yang berasal dari beberapa tutup botol yang dirangkai dengan paku sangat mengganggu telinga. Tapi tetap saja dia menyanyi kira-kira sekitar 15 menit. Saya sangat miris saat melihat tidak ada seorang pun yang memberi dia uang. Tapi sang pengamen ini terus mengiba, “Tolonglah, Pak...” (p!)
termasuk suara. Ratusan pengamen menjual suara di sarana angkutan umum di Jakarta setiap hari, mulai dari kereta api, bus, dan angkot. Saya sebagai pengguna alat transportasi publik di Jakarta adalah “penonton tetap” para pengamen ini.

Internet Pakai Hape
Mungkin sudah banyak yang tahu bahkan sering berselancar di internet pakai hape. Tapi menurut pengalamanku sendiri, banyak dari mereka yang hanya tahu kalau aktivitas ini pasti menghabiskan banyak biaya alias memboroskan pulsa telepon. Intinya, banyak orang yang belum tahu kalau internetan pakai hape itu BISA DIPAKAI dan BISA MURAH juga.
Saya sendiri juga belum lama tahu tentang ini. Ya, memang sih saya juga baru punya hape yang bisa dipakai main internet. Ya, lumayanlah. Hape saya sony ericsson K800i.
Dengan hape ini, saya bisa chatting, buka email, dan browsing dengan tarif yang sangat murah. Main sepuasnya, biaya tidak sampai seribu rupiah. Selain itu bisa mobile alias internetan dimanapun asal ada sinyal.
Ini mungkin cara internetan paling murah saat ini untuk orang awam di Indonesia. Ya, walaupun tidak seenak main di komputer atau laptop, tetap saja ini benar-benar bermanfaat. Saya bisa buka e-mail setiap saat, YM-an sama teman, lihat-lihat berita terbaru, membuka internet banking mandiri dan banyak lagi. Itu semua dengan tarif MURAH.
Saya ingin berbagi sama anda tentang internetan murah ini. Mungkin sudah banyak yang tahu, tapi mudah-mudahan yang belum tahu bisa tercerahkan.
Adapun syarat untuk bisa memakai fasilitas ini sebenarnya mudah saja. Yang pertama, anda harus punya hape layar warna yang memiliki fasilitas gprs. Yang kedua anda harus punya sim card yang operatornya mengenakan tarif gprs yang murah. Untuk saat ini, kayaknya im3 yang paling murah. Tarifnya cuma Rp. 1,1 /1 kb. Bandingkan dengan kartu lain yang tarifnya hampir 12 kali lipat. Tapi berita terbaru sih bilang AXIS lebih murah. Cuma Rp 0,1/kb.
Setelah memiliki keduanya, setting dulu gprs im3nya.Caranya ketik GPRS(spasi)merk hp(spasi) tipe hp. kirim ke 3000.Nanti akan datang konfirmasi berupa password dan setting gprs. Ikuti saja apa yang diperintahkan dalam pesan tersebut. Setelah itu arahkan settingan internet anda ke indosatgprs.
Nah sekarang sudah bisa deh. Pakai saja fasilitas browsing yang ada dihape anda. Biasanya dengan gambar bola dunia. Untuk lebih cepat lagi, saya sangat menyarankan anda memakai software OPERA MINI.
Caranya mudah saja. Buka dulu dari browser anda situs google. Kemudian search opera mini. nanti anda akan diarahkan ke situs untuk mendownload software tersebut. Biasanya sih pakai kata kunci "download opera mini". Ada dua pilihan opera mini yang ada sekarang. Opera mini versi 4 dan versi 3. Kalau versi 4 tidak bisa didownload, coba versi 3. Dengan opera mini, dijamin browsing anda jauh lebih cepat dan mudah.
Bagaimana kalau kita mau chatting? Ini juga mudah. Caranya, install dulu software YMtiny atau Yamee. software ini bisa didownload di www.getjar.com
Adapun kalau mau akses e-mail, khusus buat email yahoo, akan sangat membantu kalau anda memakai software Yahoo Go! Dengan ini, anda tidak perlu waktu lama dengan harus membuka situs yahoo.com terus masuk ke e-mail dan seterusnya. Tapi begitu anda membuka aplikasi ini, anda bisa langsung melihat e-mail anda dengan cepat.
